Medical Acupuncture offers the new patient intake form online so that it can be completed in the convenience of your own home or office.

Please print and complete the form prior to your visit. 

Your visit will not be extended and end at the allotted time. So, it it very important that the new patient intake form is completed BEFORE your scheduled appointment time. 

If you don’t have access to a printer, please arrive 40 minutes prior to your scheduled appointment time to allow yourself enough time to fill out the form.
